Northeast Health Center Group Visits for Patients Living with Type 2 Diabetes

Are you or a loved one living with Type 2 Diabetes? 

Join our four-week group visit series 10:00 AM - 12:00 PM Fridays, May 2-May 23, 205.

Our Northeast Health Center, located in the heart of Portland’s vibrant King neighborhood, will be offering weekly group visits for adult patients living with diabetes. Group visits are a great way to:

• Connect with and get support from others living with Type 2 Diabetes
• Increase understanding of how diabetes impacts your body and mental health in the short and long term;
• Learn about different medications and how they help improve diabetes
• Learn about how food can help manage your diabetes
• Learn about ways to prevent diabetes related complications 

Transportation and a healthy snack provided. Participants must be current Primary Care patient at Northeast Health Center. Group visits will include other patients, and you will be asked to provide consent to sharing health information in a group setting. To learn more talk with your Northeast Health Center provider.
