What you need for Medicaid
We can help you figure out if you are eligible for Medicaid, also called the Oregon Health Plan.
To help you sign up, we will need you to provide:
- An estimate of your income
- Your Social Security Number
- Proof of identity
- Citizenship or immigration information for everyone who needs insurance
- Any current health insurance
- Proof of pregnancy (if pregnant)
- Proof of your address (mail)
