Get access to your medical records

Find out how to request your medical records from the Health Center. You can also learn how to share your records with others.

View records on Multnomah County MyChart

Follow these steps to view your records on Multnomah County MyChart or share them with other providers, clinics, or family members. Sign up to get started.  

Go to Main Menu

Sign in to Multnomah County MyChart. In the Main Menu, tap on “Sharing.”

Tap on “Share my record”

This will take you to the Sharing Hub.

Choose how to share

Select “Yourself” to view records. You can also choose others to share with.

Other ways to get your medical records

You can also request your records in person, by phone, or by email. 

Visit a clinic

You can visit a clinic any time to request records in person. You will be asked to fill out a form. Get directions to a clinic near you.

Call us

If you have access to a printer, you can print out the request form yourself. Call us at 503-988-3997, and we will email the form to you.

Email us

You can also request the request form by email. Send a message to medical.records.request@multco.us, and we will email the form to you.

Share records

Your medical records are private, but you may want to share them with another provider or loved one. Visit a clinic or call us to request the form you need to share records.
