For your first visit

Welcome! Here’s how we’ll help you get set up as a Health Center patient.

New patient information

First, we’ll ask you to fill out a few forms for your health care team. Find out what you should bring to your first visit.

Payment information

We’ll also ask about your ability to pay for care and any insurance you have. Learn how we charge based on what you can pay.

What to bring to your visit

It’s important that you bring the following items with you when you visit the Health Center.

A form of identification

If available, please bring photo identification such as a driver’s license. Or, you can bring mail that shows your address.

Insurance cards

Bring your insurance card if you have one. If you’re getting medication, bring your prescription insurance card if you have one.

Your current medications

If you are taking medications, bring all of them with you. Or, you can also bring a list of your medications.
