Become a patient
We can help set you up as a patient. That makes us your primary health care provider.
We will provide care if you are unable to pay or don’t have insurance.
Care includes:
- Medical care
- Dental care
- Mental health care
- Pharmacy services
What to expect on your first visit
This visit is our chance to get to know you and understand your needs.
New patient information
First, we’ll ask you to fill out a few forms for your health care team.
Payment information
We'll talk with you about options for insurance, our sliding scale discounts, and other programs that could help with paying for your care.
What to bring with you
Remember to bring the following items with you for your first visit.
A form of identification
If available, please bring photo identification such as a driver’s license. Or, you can bring mail that shows your address.
Insurance cards
Bring your insurance card if you have one. If you’re getting medication, bring your prescription insurance card if you have one.
Your current medications
If you are taking medications, bring all of them with you. Or, you can also bring a list of your medications.
